Mail the following items to us at PO Box 709, Newbury Park, CA 91319-0709.

1. One copy of each player's registration form
2. A check payable to AYSO Region 42, in the amount of $150 per player. Do not send credit card numbers or cash. Checks will not be deposited until players have been placed on teams.
3. For players who are new to Region 42, please include a copy of their birth certificate or passport so that we can verify their age and ensure placement in the appropriate age division. Do NOT send the original.

Registration is not complete until all forms and payment are received, including birthdate verification. Online pre-registration is not enough to confirm a spot on a team.

A skills evaluation is required for players born August 1, 2006 or earlier (U10 and older divisions), who did not play in Region 42 last season. You and your player(s) will be contacted for a field evaluation. This is not a try-out; it is to ensure balanced teams.
